The Canadian Ophthalmological Society (COS) is proud to announce Dr. Leonard Smith as the recipient of the 2026 Humanitarian Service Award.

For more than 40 years, Dr. Leonard Smith has been the only full-time ophthalmologist serving Canada’s Territories—providing comprehensive medical and surgical eye care to some of the most remote communities in the country.

Originally from Alberta, Dr. Smith moved to Yellowknife in 1985 to serve the Northwest Territories and the Kitikmeot region, now part of Nunavut. What began as a professional opportunity became a lifelong commitment. For four decades, he has delivered truly comprehensive ophthalmic care, including cataract surgery, glaucoma surgery, strabismus repair, eyelid and lacrimal procedures, retinal laser treatments, and intravitreal injections—ensuring Northern patients can receive advanced, sight-saving care close to home.

Understanding the geographic realities of the North, Dr. Smith maintained the already established Northern Outreach Program that brings refractive and medical ophthalmic services directly to 29 remote communities across the Territories. Alongside highly trained Certified Ophthalmic Medical Technicians (COMTs) and Northern opticians, he travels to communities such as Inuvik, Hay River, and Fort Smith, reducing the need for costly and disruptive medical travel. Patients requiring surgery are then transported to Yellowknife for timely care.

For decades, Dr. Smith has led one of Canada’s only COMT training programs, personally training nearly 30 technicians. Under his mentorship, these professionals developed advanced diagnostic and clinical skills, effectively expanding access to quality eye care across the North. He also ensures ongoing education by organizing annual ophthalmic conferences for his team and advocating for up-to-date clinical and operating room equipment.

Always forward-thinking, Dr. Smith was an early adopter of tele-ophthalmology in collaboration with retinal specialists in Edmonton. This innovative program allowed Northern patients to access retinal expertise without leaving the Territories. During the COVID-19 pandemic, when travel restrictions threatened access to care, he successfully advocated for a visiting retinal specialist to provide treatment in Yellowknife—protecting patients from preventable vision loss while significantly reducing medical transport costs.

Beyond his clinical leadership, Dr. Smith is an exceptional mentor and role model. He regularly

hosts medical students and ophthalmology residents, offering not only rigorous clinical training but also an immersive introduction to Northern life. From community clinics in remote towns to experiencing the land, culture, and beauty of the North, learners leave with a deep appreciation for rural and Indigenous medicine.

Although he has completed the active phase of his career, Dr. Smith has delayed retirement to ensure Northern communities are not left without ophthalmic care. Humble, dedicated, and deeply committed to his patients, he hopes that recognition of his work will inspire the next generation to serve in Northern medicine.

Dr. Smith’s legacy is one of continuity, compassion, innovation, and unwavering service to Canada’s most remote communities.
